Scuttlebutt is that brakes failed somewhere on the way down the 4% grade on the climb down through Bolan Pass on the way fro Quetta to Rawalpindi, causing the derailment.

Quetta is near the Afghanistan border at Spin Boldak in Balochistan province of Pakistan. Rawalpindi is the legacy city adjacent to the new capital city of Islamabad. The train was just a couple of hours out of Quetta when it met with the accident.
