Recognize an Employee - unable to submit

I am sorry if this is apart from 'amtrak' discussion and more of the website issues.
I had a great room attendant on the AutoTrain last week and I wanted to enter a form stating the things she did well.
When I fill out the form and try to hit the submit button - nothing seems to happen.
I have tried on several different browsers - without luck.
I forgot to ask her last name - so I have left it blank - or put her first name in again..... Could that be the problem

Now the funny thing would be that all my submissions did go in - and I just did not get feedback - but I doubt that is the case.

The form drives me crazy. Many employees don't like to give out their last names, even more so in the era of Social Media.

I'd just put as much information as I could if I didn't have a last name. I presume Amtrak keeps enough of a record of the crew onboard a specific train for at least a while after the train has arrived they could figure out from position alone?

Just put "Unknown" in the Name block.
